Our Partnerships

All individuals wishing to access Forward House supports and services will be referred to Developmental Services Ontario to apply. The DSO is the access point for adult developmental services funded by the Ministry of Children, Community and Social Services (MCCSS) in Ontario.

The DSO South West Region is administered by Community Services Coordination Network. All individuals who request to access funding will need to complete an Application for Developmental Service and Supports and then a Supports Intensity Scale (an assessment tool, specifically designed to measure the type of support, frequency of support, and intensity of support an individual needs to fully participate in community life). When Forward House determines we have capacity to offer residential or supported independent living services we refer to Community Service Coordination Network who then identify the individual(s) with the greatest needs who have been prioritized for service.

The Passport Program, funded by the Ministry of Children, Community and Social Services, helps adults with a developmental disability be involved in their communities and live as independently as possible by providing funding for community participation services and supports, activities of daily living and person-directed planning. The program also provides funding for caregiver respite services and supports for primary caregivers of an adult with a developmental disability. Once approved for these self-directed funds, an individual may choose an agency to provide service or administer their funds.
