Accessing community services

  • Psychiatric or Psychological Services
  • LHIN Services (Physiotherapy, Occupational Therapy,
    Nursing, Dieticians, Speech Language Pathologists)
  • Legal Services
  • Financial Services (including ODSP, PGT)
  • Medical and Dental Services
  • Recreational or Employment Partners

Individuals may choose from different types of supports and services delivery models:

Supported living: For individuals who want comprehensive daily shared supports in small group settings in their homes or apartments.

Supported Independent Living: For individuals who want to live independently but desire limited supports throughout the week or month.

Passport Supports: For individuals who receive Passport monies and choose to have Forward House administer their funds, purchase support hours, or both.

Please note that funding for these models of supports are accessed through application to Developmental Services Ontario.
